Spravato Therapy

Our Clinic is a REMS approved Spravato therapy Clinic. Spravato is a form of Ketamine that is used intranasally. The FDA recommends safety monitoring while using Spravato. Spravato can be used as a standalone or it can be paired with psychotherapy. 

How Does Therapy Work with Spravato

While Spravato therapy (intranasal esketamine) offers rapid antidepressant effects through its biological mechanisms, its therapeutic potential deepens when paired with skilled psychotherapy. The experience during a Spravato session is typically more subtle and diffuse than traditional high-dose ketamine journeys. Rather than vivid inner experiences, patients often report soft shifts in perception, emotional accessibility, or unexpected clarity.

Psychotherapy during or shortly after Spravato administration helps anchor these subtle shifts. A trained therapist offers gentle guidance—helping patients notice patterns, explore emotional material, and integrate what surfaces. Sessions are conversational and attuned to the moment, using the enhanced neuroplasticity and openness that Spravato creates.

At Blossom, you can choose between a 60-minute or a 90 minute therapy session. Your therapist can also help guide you about the most appropriate duration you will need. Spravato treatments for most individuals, offer an opportunity for deeper work compared to conventional psychotherapy, especially when relational trauma, identity questions, or complex patterns are being explored.

When used with therapy, Spravato becomes more than a medical treatment—it becomes a gateway into meaning-making, change, and renewal.

Should I Do Spravato with or without Psychotherapy?

With Psychotherapy

Without Psychotherapy

 

  • Medical model focus: Emphasizes symptom reduction through esketamine’s fast-acting effects on depression and suicidality.
  • Structured dosing schedule: People attend regular sessions (usually twice weekly at the start) with medical supervision.

  • Accessible for those unable to commit to therapy: Beneficial for individuals limited by time, finances, or availability of therapists.

  • Neurobiological benefit: Works on glutamate and brain plasticity pathways to shift depressive patterns.

  • Less emphasis on narrative or emotional exploration: Sessions are supportive but not psychotherapeutic in nature.

  • Potential Insurance coverage: Your insurance might cover the cost of the medicine if you meet their criteria

Treatment-Resistant Depression (TRD):

  • For adults with major depressive disorder (MDD) who have not responded adequately to at least two antidepressant trials of adequate dose and duration.

Major Depressive Disorder with Acute Suicidal Ideation or Behavior:

  • For adults experiencing severe depressive symptoms with urgent safety concerns, where rapid symptom relief is clinically indicated.
  • Note: This indication requires inpatient or closely monitored outpatient use due to safety protocols.

FAQ

How do I start with Spravato at Blossom?

Step 1: Your clinician reviews whether you qualify as per your insurance’s criteria, measures your symptoms with a scale and refers you to Blossom

Step 2: You do a medical evaluation at our clinic in which we make sure all the information is ready to be submitted to your insurance. During that evaluation we also give you other options for your care and discuss with you about your preferences regarding therapy. 

Step 3: We add you to the Risk Evaluation and Mitigation Strategy Database. It is required by the FDA to help make sure that the benefits of the medicine outweigh the risks.

Step 4: We submit a prior authorization to your insurance and submit a prescription into your pharmacy.

Step 5: Once your insurance approves your pharmacy will drop the medicine at our clinic. 

Step 6: We will reach out to help you schedule appointment with your therapist (if you are doing psychotherapy with Spravato). If you are not doing psychotherapy with Spravato we will help you schedule appointments for your treatments at Blossom.

Step 7: At every session you will have a brief assessment, medication administration and then safety monitoring for 2hrs. You can also engage in psychotherapy in the time that you are waiting. We also help you dispose of the Spravato device safely after administration. 

Step 8: You cannot drive after the treatment so please arrange for a ride after your treatment. The manufacturer recommends driving after a night of restful sleep. 

We will help you submit a prior authorization to your insurance. Each insurance has different criteria but in general they will ask for a rating scale, previous history of medication trials and psychotherapy and symptoms that you have been struggling with. 

Week 1-4: Twice per week

Week 5-8: Once per week

Week 9 onwards: Once every 2 weeks or once a week

If Spravato is not covered by your insurance you can choose to engage in one of our other offerings for Ketamine at our clinic as per our fee schedule
